I think you need a wye instead of a laid down combo just upstream of the toilet flange.
Insulating this slab would be good, depending on climate zone. One under-slab additional consideration is electrical conduit to the service entrance, if under ground. Also a re-bar stub up to ground to, if the local code allows or requires it.

Good morning my friend, could you give me an idea o the footing for a 40×60 and 8’ tall building we are making. This will be, first God, a single room for a small church. What about your water lines? That was just rough plumbing.
how wide of a space (hole) need to be for a tub? does that space need support for the plumbing?
