I think you need a wye instead of a laid down combo just upstream of the toilet flange.
Insulating this slab would be good, depending on climate zone. One under-slab additional consideration is electrical conduit to the service entrance, if under ground. Also a re-bar stub up to ground to, if the local code allows or requires it.
